REGISTRATION FORMS

For First Time Users

Note: If you previously created a family account for a student athlete PLEASE do not create a new family account.  To register the same student, add a sibling or add a new student, login to your family account at SHS.  DO NOT create duplicate accounts.

Complete the online registration forms. 

To register the same student;

1. Login to your family account.
2. Click "Register" link (blue paper and pencil icon).
3. Choose "Register (followed by the name of the student)" from the dropdown.
4. Complete all online registration forms. 
5. Click "Submit" at the bottom of the last page.

* If you can't remember your login details, Please use the 'Forgot password'  link to get a password reset email.

Note: The form will auto populate the answers based from your previously submitted registration. Make certain to select your activity. Please review and edit the answers such as Grade and others if needed.

To add a new student in your family account:

1. Login to your family account.
2. Click "Register" link (blue paper and pencil icon).
3. Choose "Register a new student" from the drop down.
4. Complete all online registration forms.
5. Click "Submit" at the bottom of the last page.
